The short answer

NextGen sells two different EHRs, and an app built for one won't automatically work on the other. Its FHIR APIs are read-only. Read-write work on NextGen Enterprise goes through proprietary APIs under the API Distributor program, with fees that depend on scope.

The flavors to tell apart

FlavorWhat it means for an integration
NextGen Enterprise (NGE)Ambulatory EHR and PM for larger groups. Patient Access FHIR APIs plus proprietary Enterprise APIs (NGE 5.9.0+).
NextGen OfficeCloud EHR for smaller practices (5.0+). FHIR R4 with SMART App Launch and Bulk FHIR.

How access works

NextGen runs three programs:

  1. Patient Access API Program. Self-register on the developer portal for FHIR credentials.
  2. Open Access Developer Program. For NextGen clients building apps for their own use. No fees to develop or deploy.
  3. API Distributor Program. For third-party vendors. Submit the onboarding form, sign the API terms with distributor terms, build in the sandbox, then contact NextGen's API partners team before production. Clients grant and manage access to adopted apps in NextGen's API Client Portal.

Deeper Enterprise documentation sits behind onboarding.

What it costs

NextGen states no charge to develop or use sandboxes under the Distributor program. Fees may apply by data scope, version, data categories and read vs read-write, with payment not expected until general release. NextGen states no charge for GET routes on certified technology.

Our take

Ask "Enterprise or Office?" in the first call; it decides your whole plan. For Enterprise, start the Distributor paperwork early and get read-write pricing in writing before you commit to a write-heavy product.

FAQ

Does NextGen have a FHIR API?

Yes. NextGen Enterprise and NextGen Office both expose FHIR R4 APIs for patient access, which are read-only.

Can an app built for NextGen Office run on NextGen Enterprise?

Not automatically. They're different products with different API surfaces.

Does NextGen charge for API access?

Under the Distributor program, fees may apply depending on scope and read vs read-write. NextGen states GET routes on certified technology are free.
