The short answer
Oracle Health (formerly Cerner) lets you register free and build in a sandbox. For provider and system apps, production is per customer: each health system asks Oracle to provision your app against its tenant. Oracle's Millennium FHIR R4 docs list Appointment create and patch, which is more scheduling write than most large EHRs document.
The flavors to tell apart
- Millennium, Cerner-hosted. The customer's tenant ID comes from Cerner Central.
- Millennium, remote-hosted and global. Tenant IDs come from Oracle Health Hosting, with regional production hosts (APAC, Canada, UK).
- Shared-domain products (CommunityWorks, PowerWorks, Continuum) sit outside the standard provisioning flow except by exception.
- Soarian Clinicals is covered by the consumer FHIR registration path.
- DSTU2 is retired. Build on R4.
How access works
Consumer (patient) apps: accept the API terms, register in the Developer Program console, test in the sandbox, and Oracle provisions you to active consumer FHIR endpoints.
Provider and system apps:
- Accept terms, document a gap analysis, register in the console and build a working sandbox demo.
- Your app becomes available for Developer Program customers to consider.
- A customer files service requests asking Oracle to provision your client ID on its tenant, then completes its own setup.
- Custom scopes need a contract with Oracle Health.
Oracle PartnerNetwork (OPN) offers optional validation and value-added services. Oracle says you can connect without it.

What it costs
Oracle's API terms say the APIs are free in the developer sandbox; production fees, if any, are set in a separate written agreement. OPN membership and its services carry published fees that Oracle says are not required to develop or deploy.
Our take
Oracle Health's documented write surface is better than its reputation. Its access model is enterprise: no customer, no production. Line up the health system's IT and Oracle service request process before you build, and confirm which scopes and resources that tenant will actually grant.
FAQ
Is Cerner the same as Oracle Health?
Yes. Oracle completed its acquisition of Cerner in June 2022 and renamed the EHR business Oracle Health. Many developer tools still carry Cerner names.
Can you book appointments in Oracle Health through FHIR?
Oracle's Millennium R4 docs list Appointment POST and PATCH. Availability depends on the scopes each customer grants.
Do I need to join Oracle PartnerNetwork?
No. Oracle says developers can connect without OPN validation; OPN adds value-added services and marketplace listing.
